a photo of your subject(3 1/2 x5 or larger)
whether you want the picture to be on the shirt front or back
the quantity of t-shirts you will need
any special instructions concerning your photo (special cropping, etc.)
full payment (made out to Ditto's, Inc.). GA residents add 7% tax.

You provide the picture(s) and choose the month and year in which you want the calendar to begin. Please label the back of the pictures with the month in which you wish them to appear. Makes a great gift for all year 'round.
